If you have a number of changes please click on
the icon below to download the opportunity details form
(in PDF format). Fax or post the completed forms back
to us. Please download and refer to the accompanying
guidance notes (pdf) to help you.

will arrange to visit you and complete the forms together.
Remember you need to complete
one form per opportunity - so that volunteers can more
quickly and easily find the opportunities that interest
them, and we can get a more accurate idea of the demand
for volunteers.
